If you are a foreign publisher and are the recipient of US-sourced payments, use this form to certify that you are a non-US person or entity and claim the benefit of reduced rates under the applicable tax treaty.

Use to apply for a licensing account with HFA.

If you are a licensee making less than 2,500 copies, you do not need to set up an HFA Licensing Account. We recommend you complete your licensing transaction through our online service Songfile. If you are making more than 2,500 copies, please open an HFA Licensing Account using the Application for New Licensing Account. If you are making more than 10,000 copies, and have already opened an HFA Licensing Account or are an affiliated publisher that wishes to notify us of an unlicensed recording, please use this form.

Electronic Mechanical Royalty Reporting

We recommend that licensees reporting many lines of mechanical royalties submit their quarterly reports electronically to expedite processing. Acceptable formats are ASCII text, in delimited or fixed width format (.txt), MS Excel spreadsheet (.xls), or standard database format (.dbf).
