If you are aware of a current or future use of your copyrighted musical composition on a recording that has not yet been licensed, and would like HFA to attempt to license this use, please download and complete the standard mechanical licensing form below with the following information (use one form for each song to be licensed):

  1. Song title
  2. Songwriter's name(s)
  3. Publisher name and copyright percentage splits
  4. Record company name and address (if you have a contact name /or phone number, this would also be helpful)
  5. Catalog number (usually on the spine of a recording) and configuration (CD, LP, Cassette, etc.)
  6. Playing time for each selection (especially if the song is over five minutes in length)
  7. Release date
  8. Artist name
  9. Title of album
  10. Sign your name in the "Publisher's Approval" section

When completed, please return the form(s) to Publisher Services, and we will strive to obtain the licenses as soon as possible. Should you have any questions, please contact a Publisher Services representative at 212-834-0100, or publisherservices@harryfox.com.

Please note:   HFA only licenses recordings manufactured and distributed in the United States, its territories and possessions.
  * For recordings manufactured outside of the U.S., and imported into the U.S., please contact a Publisher Services representative for further instructions.
  * For recordings both manufactured and distributed outside of the U.S., if you have authorized HFA to act as your representative in that territory, please use the Notice of Foreign Activity form.
